There are two rail connections between Romania and Bulgaria, both cross the Danube on a large bridge: from Giurgiu to Ruse, where the train from Bucharest to Sofia runs and from Calafat to Vidin.

If you travel via Serbia, travel from Sofia to Nis, then from Nis to Skopje. If you want to travel via Greece, you have to travel from Sofia to Thessaloniki then from Thessaloniki to Skopje.
However as both options require many hours of travelling you might instead choose to go by bus. Buses are available from Sofia directly to Skopje and are much quicker than the available options by train. Buy your ticket locally at the bus station.
